MUSCLE SCRAPING:
PRECISION TISSUE WORK
When scar tissue and fascial restrictions won't respond to hands-alone therapy, specialized instruments unlock what nothing else can reach.
WHAT IT IS
MORE THAN JUST AN ADJUSTMENT
Muscle scraping — the name we use in our clinic because it accurately describes the sensation and action — is formally known as IASTM (Instrument-Assisted Soft Tissue Mobilization). You may also know it as Graston Technique or Gua Sha, two of the most recognized formal systems within the IASTM family. All use specially designed tools to detect and treat soft tissue dysfunction in a way that bare hands simply cannot replicate.
WHY USE A TOOL INSTEAD OF YOUR HANDS?
- The instruments act as a mechanical extension of the practitioner's senses and force. They amplify tactile feedback, allowing us to precisely detect areas of scar tissue, fascial restriction, and tissue disorganization that might go unnoticed through palpation alone. They also allow us to deliver very controlled, targeted force to structures at depths and angles that manual therapy can't efficiently access. Think of it as the difference between locating a splinter by feel versus using a magnifying glass.
WHAT TO EXPECT DURING TREATMENT
- We apply emollient (oil or cream) to the skin to allow smooth instrument movement. You'll feel a scraping or stroking sensation — the pressure is significant but should stay within a "productive discomfort" range. You may experience redness (petechiae) during and after treatment — this is expected and typically resolves within 1–2 days. Most clients feel a noticeable change in tissue mobility immediately after a session. We typically follow IASTM with movement work to reinforce the tissue changes while the window of opportunity is open.
THE SCIENCE BEHIND IT
- IASTM creates controlled microtrauma in restricted tissue, triggering your body's healing cascade: increased fibroblast activity, improved collagen fiber remodeling, and enhanced blood and lymphatic flow. The result is that disorganized, restrictive scar tissue is broken down and replaced by healthy, aligned collagen — restoring tissue extensibility, reducing pain, and improving function.
WHAT THE RESEARCH SHOWS
- IASTM has strong clinical evidence supporting its use for tendinopathies, plantar fasciitis, scar tissue management, carpal tunnel syndrome, and post-surgical rehabilitation. Graston Technique specifically has been adopted widely by professional sports organizations, Division I athletic programs, and military rehabilitation centers.
WHAT WE TREAT WITH MUSCLE SCRAPING
- Achilles tendinopathy and plantar fasciitis
- IT band syndrome and lateral knee pain
- Rotator cuff tendinopathy and shoulder adhesions
- Tennis elbow and golfer's elbow (lateral/medial epicondylitis)
- Post-surgical scar tissue (once fully healed)
- Shin splints (tibial stress syndrome)
- Chronic low back myofascial restrictions
- Hip flexor and TFL tightness in runners and cyclists
